OEF APPLE Award Order Form

Tell us exactly how you want your award to read. Submit this form for each requested award. When finished, go to the "payment" tab on the menu to pay for your awards.

The OEF is proud to announce that _______ (Who is giving this award? List name as you want it printed on the award) *

is honoring ______ (list name of person to be honored, as you want it printed. Also include the name of school, although this will not be printed on the form). Ex: Mrs. Smith (Kinawa) *

for _____ (List the reason for the award, exactly how you want it listed on the form). If nothing is listed, award will read "for making a difference in Okemos Schools." *

Submit Award Order

After submitting your award, go to the menu (upper left corner, 3 bars) and either order another APPLE Award or Process Payment for your Award(s). Thank you!
